Monthly: 1- Fellow evaluation by Attending 2- Attending evaluations by Fellow (anonymous- turn in to program director office in sealed envelope, due every 6 months) 3- Duty hours monitoring
Quarterly: 1- Fellow evaluation by Staff (nurse or tech) 2- Fellow evaluation by Patient
Yearly: 1- Fellow evaluation by Program Director
